
As an American Red Cross Authorized
Provider Instructor, you can leave the administrative chores to
us. Your local Red Cross chapter provides administrative
support, including issuing course certificates and maintaining
instructor and student records, so that you can focus on what’s most
important-providing lifesaving training. In order to maintain
accurate student and instructor records, we depend on you to provide
completed course activity reports to us here at your local Red Cross
Chapter within 10 business days following course completion.
These reports are critical to the success of both your program and
ours, and help verify the number of students and classes each
certified instructor has taught.

Upon receipt of your training records the Red
Cross will confirm receipt with a reply e-mail. Student
certificates can be picked up two days after receipt of training
records in Health & Safety Services Department. Office
hours for certificate pick-up are 8:00 AM – 4:00 PM Monday
–Friday.
INSTRUCTOR REMINDERS!
Please complete and submit the
following required paperwork to the Health & Safety (H&S) Department
when returning the Red Cross equipment:

* Please complete the Activity
Report form and Addendum thoroughly indicating which training
components students have completed and indicating a grade of P for
Pass, F for Fail, I for Incomplete, or A for Audit. Criteria for
grading is outlined in your instructor’s manual.
* We encourage all students to use
their professional name and use care in writing legibly, the
information they record on their Registration Form will be
transferred to the Activity Report Form and than processed into our
computer data system. When servicing a student request for a
duplicate certificate information can be located and processed
quicker having accurate information.
* Complete the
Activity Report Form and Addendum noting each student’s home mailing
address, phone number, and email address. Duplicate certificate
requests are verified using the student’s personal information and
if names are duplicated in our data system (example: Ann Smith is
requesting a duplicate card and we may have 10 Ann Smith’s listed)
we can fulfill the request quicker and with accuracy.
Note: Please
conduct the 5 minute Be Red Cross Ready Presentation & indicate the
HSBRR104 code on your course record.

CHECK ALL EQUIPMENT
BEFORE RETURNING TO HEALTH & SAFETY DEPARTMENT
-
Return all equipment so it is “check-out ready” for the next instructor
to use.

Check AED trainers - each pad (adhesive side) should be placed on the
shiny side (inside) of fold out-make it a student task to replace
the pads correctly.
-
Place the AED trainer back in the case correctly with the washcloth on
the top of trainer-make it a student task.
-
If batteries in an AED trainer need to be replaced make H&S aware.
-
Check First Aid Tub-refold blankets and contents- make it a student task.
-
The manikin face packets-RETURN ALL FACE PIECES TO RED CROSS-THE USED AND
UNUSED (keep separated)-YOU WILL BE CHARGED AN EXTRA FEE OF $7.50
FOR EACH FACE PIECE NOT RETURNED OR RETURNED WITH MISSING CONNECTOR.

Dispose of all used gloves, training shields, and alcohol wipes-make it a
student task. Return all unused training items to Health & Safety.

RETURN ALL EQUIPMENT TO HEALTH & SAFETY PROMPTLY ON YOUR ASSIGNED RETURN
DATE TO ELIMINATE A LATE FEE BILLED TO YOU OR YOUR COMPANY!
